Post-Surgery Shoulder Pain: What to Expect in Recovery

Shoulder surgery can be life-changing, but it’s important that your recovery is handled with care. Many patients wonder what the healing process involves and how therapy fits into it. At Protailored Physical Therapy, we guide patients through post-surgery shoulder recovery. By combining physical and occupational therapy, we help people regain strength, mobility, and confidence.

We want to share Mark’s story to help you better understand post-surgery rehabilitation. Mark (not his real name) recently worked with us after a rotator cuff repair. Here’s what his rehab looked like.

From Surgery to Strength: Mark’s Road to Shoulder Recovery

When Mark tore his rotator cuff after years of playing golf, surgery was just the beginning. He chose Protailored Physical Therapy for his rehabilitation, and with a combination of physical and occupational therapy, he made a full return to activity in just under six months.

His experience offers a powerful example of what post-surgery shoulder recovery can look like when therapy is done right.

Early Recovery: Setting the Foundation for Healing

The first few weeks of Mark’s shoulder surgery recovery focused on reducing pain and inflammation, protecting the surgical site, and preventing stiffness through passive motion exercises.

He began physical therapy within the first week after his surgery. He was already wearing a sling to keep his shoulder immobilized and protect it during day-to-day life. However, during his sessions, one of our physical therapists physically guided him through gentle passive range-of-motion movements to promote circulation and prevent frozen shoulder.

At the same time, one of our occupational therapists helped Mark adjust his daily activities so he could safely bathe, dress, and perform basic self-care without strain. These sessions supported his independence while his body healed.

Regaining Motion: Gentle Movement and Active Participation

Once his surgeon cleared him for exercise around the four-week mark, Mark moved into active-assisted exercises:

  • Pendulum swings and pulleys to encourage range of motion
  • Stretching to reduce tightness and improve joint mobility
  • Therapeutic activities to restore upper-body control

We monitored his form closely to ensure he wasn’t compensating with his neck and spine, which could impede his recovery.

Mark also worked with one of our occupational therapists to begin relearning fine motor skills and strategies for reintegrating everyday arm movements. We showed him some adaptive strategies he could use to return to office work.

During this stage of post-surgery shoulder recovery, Mark started to feel more like himself again, and it wasn’t long before he could lift his arm above shoulder height without pain.

Building Strength: A Return to Confidence and Activity

By the third month of his rehabilitation, it was time to begin restoring strength and function. We focused on progressive strengthening exercises using resistance bands and light weights. These exercises targeted his rotator cuff and scapular stabilizers. We also guided him through postural corrections to support long-term joint health.

Meanwhile, Mark’s occupational therapist customized tasks that mimicked typing and golf swings to retrain his coordination and power.

By the end of his rehabilitation program, Mark was able to return to the office and the golf course.
