At ProTailored Physical Therapy, we want to maximize your time with us. If you would like to save time on your first visit, please print the following patient documents below, fill them out, and bring them with you to your first appointment. If you are a new patient, please print and fill out the New Patient Intake Form and bring to your first appointment. If you are a previous patient of ours, please print and fill out the Returning Patient Intake Form and bring to your first appointment. You may also arrive twenty minutes prior to your first appointment to fill out your paperwork here. Roadmap to Recovery

Coming into your first appointment, you may have questions floating through your mind such as, “When will this pain go away? What is causing my pain? Do I need surgery? How many appointments should I expect to have? Will I be able to accomplish my goals and return to normal functioning? What kind of treatment techniques will I receive during my sessions?” These are all good and normal questions to have. Our goal is to answer as many of these questions during your first appointment as we can.

When you arrive at ProTailored Physical Therapy, you will receive your personalized “Roadmap to Recovery” folder. Contained inside will be a goal sheet to write out goals you want to accomplish through physical therapy, charts explaining the different healing stages your body will go through, an estimate of how long full recovery will take, and space for your individualized plan of care that your physical therapist will expertly craft and tailor to you and your specific needs.

During your first appointment, you will meet and get to know your physical therapist, and your physical therapist will get to know you. After listening to your story, history, and goals, your physical therapist will do a thorough physical assessment to find the root of the issue by discovering the cause and where it is originating. Taking into account your history and goals as well as what your physical therapist observes in their physical assessment, your physical therapist will craft your individualized plan of care specifically aimed to get you better. Your individualized plan of care will detail delivering advanced treatment techniques, prescribing exercises, and imparting specific knowledge that will empower you to heal now and prevent re-injury later.  You will leave your first appointment with a solid plan in place to get you better. You and your physical therapist are a team; they will walk alongside you every step of the way-encouraging you, listening to you, and seeing you through your full plan of care.
